For generations, childhood complaints of a severe headache have too often been dismissed as a fleeting excuse to skip school or put off homework. But a leading doctor is pushing back against that old-school skepticism. Dr Sudhir Kumar, a neurologist at Apollo Hospitals in Hyderabad, took to X on August 24 to outline a comprehensive framework on pediatric migraines. According to him, the condition is a genuine neurological disorder that demands clinical validation rather than casual dismissal. Drawing from medical science and clinical experience, Dr Kumar shared ’10 things every parent should know about migraine in children’, shedding light on a condition that he said affects roughly 10 percent of children.

Understanding the reality of chilodhood migraines

Migraines are far more common in younger populations than many realise, according to Dr Kumar: “Migraine affects approximately 10 per cent of children, and its prevalence increases with age.” About half of all sufferers experience their initial attack before turning 12, he added.

Furthermore, genetics plays a heavy role. Noting that ‘migraine often runs in families’, Dr Kumar explained: “A child with recurrent headaches is more likely to have migraine if a parent or close family member has it. Unlike adults, who can pinpoint throbbing pain and sensory auras, younger patients may experience different symptoms. Dr Kumar pointed out: “The headache is more often bilateral [on both sides] and may be shorter, typically 2–72 hours.” He added, “Young children may not describe light and sound sensitivity but may simply want to lie down in a dark, quiet room.”

Moreover, the clinical picture extends well past head pain. “It is not always ‘just a headache,'” Dr Kumar cautioned, noting that nausea, vomiting, abdominal pain, dizziness, and sudden paleness or unusual quietness frequently accompany attacks.

Management, triggers and medication safety

When managing attacks, timing is everything. Dr Kumar advised parents to ‘treat the attack early’, noting that weight-appropriate doses of ibuprofen or paracetamol — or specific triptans for moderate-to-severe cases, under strict medical supervision — are significantly more effective when administered at the onset.

However, he issued a strict warning against overmedication: “Frequent use of medicines for headache can itself contribute to more frequent headaches.” To prevent medication-overuse headaches, simple analgesics should be limited to 14 days or fewer per month, and triptans should be restricted to a maximum of nine days per month, he highlighted.

Lifestyle adjustments remain central to long-term control. Dr Kumar stressed that ‘prevention is not just about medicines’, pointing out that regular sleep schedules, consistent meals, hydration, and stress management make a profound difference. While common triggers include missed meals, inadequate sleep, stress, dehydration, and excess caffeine from chocolates or soft drinks, he advised against overly restrictive diets unless a specific trigger is definitively identified. Long-term outlook and recognising red flags

While childhood migraines can persist into adulthood — particularly among girls — the long-term trajectory is generally hopeful. “The good news is that frequency and severity often improve over the years,” Dr Kumar said. Crucially, not every childhood headache is a migraine, but parents must remain vigilant for dangerous warning signs.

Dr Kumar urged families: “Seek urgent medical attention for a sudden, severe ‘worst-ever’ headache, headache with weakness or altered consciousness, seizures, persistent vomiting, abnormal neurological symptoms, or a major change in the usual headache pattern.”

Dr Kumar concluded: “Migraine is a real neurological disorder; not an excuse to avoid school or play. It can affect education, friendships and quality of life. The good news is that with the right diagnosis, healthy routines and appropriate treatment, most children with migraine can lead completely normal, active lives.”
